  • Address: Complete residential address.
  • Phone Number: Contact number.
  • Email Address: Email address (if applicable).
  • Occupation: Profession or occupation.
  • ID Proof: Mention any ID proof submitted (e.g., Aadhaar, Passport).

3. Details of the Opposite Party (Service Provider or Seller)

  • Contact Details: Phone number and email address.
  • Name of the Responsible Person: If applicable, mention the name of the person (like the manager or CEO) responsible for customer service or complaints.

4. Nature of the Complaint (The Issue)

  • Details of the complaint: Explain the nature of the problem, such as defects, substandard service, poor quality, wrong billing, non-delivery, etc.
  • Date of the incident: Mention the date when the issue occurred or the service was rendered.
