A formal name change is generally governed by administrative procedure and is supported by publication in a gazette under government rules. 1. Affidavit for name change This is the most important document. It is a sworn statement declaring: Old name New name Reason for name change Personal details (address, age, etc.) It is made on stamp paper and notarized. 2. Newspaper publication You must publish the name change in: One local newspaper (regional language) One English newspaper The advertisement must include: Old name New name Address and declaration 3. Gazette notification For legal recognition, the name change is published in the Official Gazette of India or State Gazette. This is considered final legal proof of name change. 4. Identity and address proof Common documents required include: Aadhaar card PAN card Passport (if available) Voter ID Address proof (electricity bill, ration card, etc.) 5. Application form A prescribed application is submitted to the concerned department for gazette publication or record update. 6. Passport-size photographs Usually 2–3 recent photographs are required for verification and publication. Dear Client, Getting your name legally changed in India entails certain documentations that depend on what kind of change is sought that is a legal name change for all official records or educational certificates or identity proofs. The general process involves three steps which are executing an affidavit, publishing a name change notice in an official gazette, and publishing in two local newspapers. The documents required are as follows. Firstly, you have to obtain a name change affidavit that must be sworn before either a notary public or judicial magistrate, and which should clearly state your previous name, proposed name, and reason for change. The second point is that you will have to attach your identity proofs along with the affidavit such as the aadhaar card, PAN card, and passport if you have it in your old name. The third point is that you will have to make a request for publication in the gazette of your respective state government. This process differs from state to state but involves submission of your affidavit along with a payment of fees to the concerned State Gazette office. Fourthly, you will have to publish the aforesaid name change notice in two local newspapers one english and one in the regional language as corroborating evidence. After you have gone through all these steps, make sure that you update your name everywhere else too through the Gazette notification, in combination with the news clippings and affidavit, which includes your Aadhaar through the UIDAI portal, PAN through the NSDL or UTI portal, passport through the passport office, bank accounts, driving license through the RTO, and voter ID through the Election Commission.
